This series consists of applications for enrollment in non-reservation schools. The applications were submitted on Standard Form 5-192a for admission to Flandreau School and Carlisle Indian School. Information in the applications includes the student's birth date; sex; degree of Indian blood; the results of a physical examination; the names of parents or guardians; and, in some cases, the previous academic record. The applications also include a statement of consent for enrollment of the child, signed by the parent or guardian. The file also contains case records, physical examination and health records, and certificates of promotion submitted on standard forms; and correspondence related to enrollment.

The Carlisle Indian Industrial School was the brainchild of a young lieutenant of the 10th United States (U.S.) Cavalry, Richard Henry Pratt.
